the ++/keys/values patch

Somehow since r21167 docs.lib won't build. I haven't figured it out
yet. I have a nice long dentist appt today so I can't look immediately.

Also, I changed the behavior of keys/values as described, but only
changed the annotation on ++. After I figure out what I've broken
without that change, I'll see about unbreaking that and breaking
something different.
